Policies & Health Forms <<

Medication Form - Required by state law if your child needs to take any medication at school, including over the counter medications. It must be completed and signed by a physician and by a parent/guardian. All medication must be brought to school in its original container and must match the medication order. If your child has a life-threatening condition (diabetes, severe food allergy, severe bee sting/insect allergy, severe seizures, severe asthma or other rare condition) an emergency action plan is also required before your child attends school. 

Life Threatening Illness / Condition - Law in Washington State requires that children with life-threatening conditions have medication and /or treatment plan orders on file before attending school. A life-threatening condition is defined as one that puts the student in danger of death during the school day. This includes students with severe bee sting or food allergies, diabetes, severe asthma, severe seizures and possibly some other rare conditions. This means the school must receive from you, the proper paper work, including physician orders, health care plans and /or emergency plans prior to your child starting school in the fall. This law is in place to ensure that school will be a safer and healthier place for your child. Please plan to bring the forms to the school no later than the week before classes start to allow time to consult with you, if necessary, prior to the first day of school. Call Glacier View Junior High for questions and appointments with the school nurse and/or other staff at 253-840-8922. If a medication or treatment order is not provided, the chief administrator of the school is required to exclude your child until such order has been provided.

About Mrs. Yoest <<

Mrs. Yoest earned her bachelors degree in nursing at Salisbury University in Salisbury, Maryland.  She started her career as an Army nurse.  Her nursing experience includes: cardiology, adult and pediatric intensive care neurology and cardiology nursing.  She received her ESA in school nursing through Pacific Lutheran University.  Mrs. Yoest was a Nurse at Stahl for eight years before coming to Glacier View.  She has two children attending school in the Puyallup district.
