Welcome to the Nurse's information page.

Hello!  I am Denise Vanaman, the school nurse at Edgemont.  I’m a registered nurse with a BSN from PLU and have 18 years of experience as a school nurse as well as seven years experience at Mary Bridge Children's Hospital. I am at Edgemont every Wednesday and Friday. When not at Edgemont I am at one of our three North Hill elementary schools. I am always available for consultation by telephone or email (dvanaman@puyallup.k12.wa.us).

Washington State law requires that we have physician’s orders, written parent consent and medication provided in the original container in order to administer medications (including over the counter medication like Tylenol) at school.  For students who have potentially life threatening conditions, such as diabetes, severe asthma, seizure disorders or allergies, state law also requires that a health plan and medication/treatment orders be in place in order for the child to attend school.  These plans and physician’s orders must be renewed each school year.

The Physician’s Order for Medication at School form, a generic Emergency Action Plan form as well as Emergency Action Plan forms for Seizure Disorder, Food Allergy and Asthma are available on the website.

You may check out the new web page located on the district web page that provides helpful information and forms : http://www.puyallup.k12.wa.us/studentlearning/specialservices/health.cfm
